Admission Documents

Applications and other pertinent information may be communicated between College offices. It is the applicant's responsibility to have transcripts and all other appropriate documents necessary to comply with admission requirements submitted to the Office of Admissions and Records for evaluation. Falsification or incomplete documents may be grounds for denial of admission. Admission materials will be kept on file for 12 months. If admission is not granted upon first review and the applicant wishes to use the application that was originally submitted he/she must submit a written request to the Coordinator of Admissions and Records. Admission documents including application for admission, reference forms, transcript request forms, etc. are available within this website as well as in the main office of the Danville campus and in 1443 Buzzard at the EIU campus. All documents submitted to Lakeview College of Nursing for admission or transfer purposes become the property of the College. Transcripts from other institutions will not be released to a student, nor will they be forwarded to other educational institutions or agencies unless specified by state or federal regulation. However, please be advised that LCN applications submitted by students who are admitted to the LCN/EIU Campus will be copied and forwarded to the EIU admissions office so that LCN/EIU students receive services on the EIU Campus.  A student needing copies of documents from other institutions must contact those institutions directly.
