Transcripts

While a student is currently enrolled and attending Lakeview College of Nursing transcripts are free of charge. A fee of $5.00 is charged for each transcript requested thereafter. Transcripts will be issued only upon written authorization from the student. Transcripts will not be issued until all College obligations, financial or otherwise, are met. To receive an official transcript please complete the required transcript request form and submit it to the office of the Coordinator of Admissions & Records/Registrar. You must include your full name when enrolled at LCN, any name change, social security number, dates attending the college, and current address.
