Student Records
The following student educational records are kept in the Registrar's Office at Lakeview College of Nursing: original application, college transcript(s), ATI scores, and health records. The College retains student educational records for a period of 7 years after graduation. At that time all records are discarded and the College of Nursing transcript and the original application, including all documents and correspondence in the only record kept on file.
Access to Academic Records
All information collected, assembled or maintained in an official file by Lakeview College of Nursing concerning an individual student is available to that student under the supervision of the Director of Enrollment/Registrar. Students may request copies of heath records contained in the file.
Name or Address Change
At the time of registration and throughout the student's enrollment in the College of Nursing on both campuses, it shall be the student's responsibility to update their contact information through SONIS Web.
In the event that a student's name changes, student's should contact the Director of Enrollment/Registrar. A name change must be substantiated by two forms of legal documentation such as a marriage certificate and driver's license. A copy of these documents will be placed in the student's file.
Health Records / Hepatitis B Policy / TB / Immunizations
The risk of Hepatitis B infection among health care workers is becoming ore prevalent. All incoming students must have proof of Hepatitis B vaccination, immunity, or have signed a waiver on the Student Health Reccord. Student must have a yearly TB Test, and up to date immunization verification on file in the Registrar's office.
