Identification

Student ID Cards (Danville Campus) / Panther Cards (EIU Campus) will be required of all students and made available through Provena United Samaritans Medical Center and Eastern Illinois University. Students will be required to go to Provena USMC or the Telecom Office at EIU in order to have their photo taken for use with this ID card. Students at EIU will need to acquire Panther Cards for identification and services.

All students admitted to Lakeview College of Nursing will be issued, at no charge, a name pin. This identification must be worn at all times at clinical sites or while in an LCN student role in the community. Students are required to protect their name pin from any unauthorized use. Students must notify the Director of Administrative Services immediately if their name tag is lost or stolen. Students will be charged for replacement pins.

For further identification in clinical sites, students will also be required to sign SN-LCN after their name on patient's charts.
