Regular Admission Application Form
For students who do not currently hold nursing licensure and wish to become fully admitted into LCN's BSN program. You should have all coursework required for admission complete prior to beginning the LCN Nursing Program.

Basic Nurse Assistant Application Form
For students wishing to achieve a BNA certificate. This is the first step in qualifying for BNA/CNA positions. For more information about the program look at our brochure.

RN-BSN Application Form
For current RNs wishing to become fully admitted into LCN's BSN program on the Danville Campus.  You will be required to have the appropriate coursework complete.

LPN-BSN Application Form
For current LPNs wishing to become fully admitted into LCN's BSN program.  You should have all coursework required for admission complete prior to beginning the LCN Nursing Program.

Non-Degree Seeking Application Form
For students wishing to take preliminary coursework (Chemistry, Statistics, Medical Terminology) with LCN and/or for those who are NOT wishing to pursue a degree with LCN.
